You will need copies of the following information to help you complete the form (and you need to enclose these copies with your application):

  • Your information request to the authority
  • The authority’s response to your information request (if you received one)
  • Your request for review to the authority
  • The authority’s response to your review request (if you received one)

Frequently asked questions

Do I have to use the application form?

No, but it is a good idea to use it if you can because it helps you give us the information we need. If you don’t want to use the form, you can send your appeal to us as a letter or email. See making an appeal without a form.

Should I attach other information about the case to my appeal?

Please don’t send us extra information at this point. We will ask you for any further information we need once we start our investigation.

What happens once I’ve sent the application form?

We will acknowledge receipt of your form within two working days. In most cases, we'll be in touch again very quickly to give you the contact details for the investigating officer. If there is any problem with your application, we'll also contact you quickly to discuss it with you or ask you for more information. We'll send you our Guidance for Applicants (you can download this here).
